Composition
Each tablet contains:
Nicorandil 10 mg or 20 mg
Auxiliary components:
potato starch, microcrystalline cellulose type 102, calcium stearate.
Properties
Antianginal agent, has nitrate-like properties and the ability to open potassium channels.
Due to the opening of potassium channels, nicorandil causes hyperpolarization of the cell membrane, and the nitrate-like effect is to increase the intracellular content of cyclic guanyl monophosphate. These pharmacological effects lead to relaxation of smooth muscle cells and provide a protective effect on cardiomyocytes during ischemia.
The hemodynamic effects are expressed in a balanced decrease in pre- and afterload, in contrast to nitrates and slow calcium channel blockers, which act mainly on pre- and afterload, respectively. Indications
Relief of angina attacks; prevention of attacks of stable angina pectoris (in combination with other antianginal drugs) and as monotherapy for intolerance to beta-blockers and blockers of slow calcium channels.
Method of administration and dosage:
Is taken internally. The dose is set individually, depending on the severity and duration of the disease. Single dose — 10-20 mg, frequency of use — 1-3 times / day. The maximum daily d is 80 mg.
Contraindications
Acute myocardial infarction (and within 3 months after); cardiogenic shock, collapse; unstable angina, incl. Prinzmetal’s angina; chronic heart failure of Ill or IV functional class according to NYHA classification; severe bradycardia (heart rate less than 50 beats / min); AV block Il and Ill degree; left ventricular failure with low filling pressure; severe arterial hypotension (diastolic blood pressure at rest less than 100 mm Hg); severe anemia; refractory hyperkalemia; concomitant use of PDE5 inhibitors (sildenafil, vardenafil, tadalafil); pregnancy, lactation (breastfeeding); children and adolescents up to 18 years old.
Precautionary measures:
It should be used with caution in patients with cardiac arrhythmias, with impaired renal and / or liver function, hypovolemia, arterial hypotension, pulmonary edema, angle-closure glaucoma, hyperkalemia, 1st degree AV blo and hyperthyroidism.
Side effects
From the side of the cardiovascular system: palpitations, tachycardia, decreased blood pressure, postural hypotension, flushing to the skin of the face, peripheral edema. When using nicorandil in a high dose, arterial hypotension and / or tachycardia is possible.
From the side of the central nervous system: headache, dizziness, tinnitus, insomnia.
From the digestive system: nausea, vomiting, discomfort in the stomach, a feeling of fullness in the stomach, stomatitis (extremely rare); in some cases — an increase in the activity of ALT, AST, ALP.
