Theophylline anhydrous 300 mg

Properties

Phosphodiesterase inhibitor, methylxanthine. Increases the accumulation of cAMP in tissues, blocks adenosine (purine) receptors; reduces the flow of calcium ions through the channels of cell membranes, reduces the contractile activity of smooth muscles. Relaxes the muscles of the bronchi, blood vessels (mainly the vessels of the brain, skin and kidneys); has a peripheral vasodilatory effect, increases renal blood flow, has a moderately pronounced diuretic effect. Stabilizes the mast cell membrane, inhibits the release of mediators of allergic reactions. Increases mucociliary clearance, stimulates diaphragm contraction, improves the function of the respiratory and intercostal muscles, stimulates the respiratory center. By normalizing the respiratory function, it helps to saturate the blood with oxygen and reduce the concentration of carbon dioxide; stimulates the centers of respiration. Enhances lung ventilation in conditions of hypokalemia.

Indications

Broncho-obstructive syndrome of various origins: bronchial asthma (the drug of choice for asthma of physical exertion and as an additional remedy for other forms of bronchial asthma), COPD (chronic obstructive bronchitis, pulmonary emphysema). Pulmonary hypertension, cor pulmonale, edematous syndrome of renal origin (as part of combination therapy), sleep apnea.

Dosage and administration

The optimal dosage regimen is determined by the doctor. Compliance of the dosage form of a particular drug with indications for use and dosing regimen should be strictly observed.

The maximum doses that can be used without monitoring the concentration of theophylline in blood plasma:

children aged 3-9 years — 24 mg / kg / day,

Contraindications

Epilepsy, peptic ulcer of the stomach and duodenum in the acute phase, gastritis with high acidity, recent bleeding from the gastrointestinal tract, severe arterial hyper- or hypotension, severe tachyarrhythmias, hemorrhagic stroke, retinal hemorrhage, hypersensitivity to theophylline and other xanthine derivatives ( caffeine, pentoxifylline, theobromine).

Precautionary measures:

Use with caution in severe coronary insufficiency (acute phase of myocardial infarction, angina pectoris), widespread vascular atherosclerosis, hypertrophic obstructive cardiomyopathy, ventricular premature beats, chronic heart failure, increased convulsive readiness, with liver and / or kidney failure, gastric ulcer and duodenal ulcer ( in history), with uncontrolled hypothyroidism (the possibility of cumulation) or thyrotoxicosis, prolonged hyperthermia, gastroesophageal reflux, prostatic hypertrophy, during pregnancy, during lactation, in elderly patients, in children (especially for oral forms).

Side effects

From the side of the central nervous system: dizziness, headache, insomnia, agitation, anxiety, irritability, tremor.

From the side of the cardiovascular system: palpitations, tachycardia (including in the fetus when taken in the third trimester), arrhythmias, decreased blood pressure, cardialgia, increased frequency of angina attacks.

From the digestive system: gastralgia, nausea, vomiting, gastroesophageal reflux, heartburn, exacerbation of peptic ulcer, diarrhea, with prolonged use — loss of appetite.
