Properties
Carvedilol is an antihypertensive agent from the group of alpha- and beta-blockers without internal sympathomimetic activity. Blocks α1-, β1- and β2-adrenergic receptors. As a result of the blockade of β1-adrenergic receptors, it moderately reduces the conductivity, strength and frequency of heart contractions, without causing a sharp bradycardia. As a result of the blockade of α1-adrenergic receptors, it causes the expansion of peripheral vessels. As a result of the blockade of β2-adrenergic receptors, it can slightly increase the tone of the bronchi, some vessels of the microvasculature, as well as the tone and peristalsis of the intestine.
Hydrochlorothiazide is a thiazide diuretic. Violates the reabsorption of sodium, chlorine and water ions in the distal tubules of the nephron. Increases the excretion of potassium, magnesium, bicarbonate ions; retains calcium ions in the body. The diuretic effect occurs after 2 hours, reaches a maximum after 4 hours and lasts up to 12 hours. It helps to reduce high blood pressure.
Indications
Arterial hypertension.
Dosage and administration
Taken inside. The initial dose is 1/2 tablet 1 time / day, then the dose is increased to 1 tablet 1 time / day.
Contraindications
AV block II and III degree, severe bradycardia, heart failure in the stage of decompensation, shock, bronchial asthma, chronic lung disease with a history of broncho-obstructive syndrome, liver failure, pregnancy, lactation, hypersensitivity to carvedilol, anuria, severe forms of gout and diabetes diabetes, refractory hypokalemia, hypercalcemia, hyponatremia.
Warnings and precautions:
Use with caution in patients with coronary artery disease and heart failure. Before starting treatment with carvedilol in heart failure, adequate therapy should be carried out to eliminate the symptoms of decompensation.
Side effects
From the side of the cardiovascular system: at the beginning of treatment (the effect of the first dose) and with increasing doses, episodes of excessive reduction in blood pressure are possible (usually these phenomena go away on their own and the patient’s condition stabilizes without special correction); with further use of carvedilol, bradycardia is also possible; less often there are violations of peripheral blood flow, angina attacks, AV blockade, episodes of intermittent claudication; in some cases — increased manifestations of heart failure.
From the side of the central nervous system and peripheral nervous system: weakness, fatigue, headache are possible; in some cases — sleep disturbances, mental depression, paresthesia.
From the digestive system: nausea, vomiting, increased tone and intestinal motility; in some cases — a change in the activity of transaminases in the blood plasma.
