Composition

Isosorbide 5- mononitrate 50 mg

Inactive ingredients:

Sugar spheres(corn starch and sucrose), lactose monohydrate, poly(o-ethyl) cellulose, talk, hydroxypropyl cellulose, polyethylene glycol 20 000, gelatin, titanium dioxide E 171, iron oxide red E 172, iron oxide black E 172.

Properties

Like all organic nitrates mononitrate acts as a donor of nitric oxide. NO causes a relaxation of vascular smooth muscles via the stimulation of guanytyl cyclase and the subsequent increase of intracellular cyclic guanosine monophosphate concentration.

A cGMP dependent protein kinase is thus stimulated, with resultant alteration of the phosphorylation of various proteins in the smooth muscle cell. This eventually leads to the dephosphorylation of the light chain of myosin and the lowering of smooth muscle tone.

Indications

Long-term treatment if coronary artery disease

Long-term treatment of angina pectoris ( including post myocardial infarction)

Long-term treatment of criminal congestive heart failure in combination with cardiac glycosides , diuretics, ACE — inhibitors or arterial vasodilators.

Dosage and administration

-All dosage forms should be swallowed whole with water.

-The lowest effective dose should be used.

-Dosage regimen should be designed according to the clinical response of the patient.

Side effects

The most common:

-Headache: The incidence of headache decreases rapidly with continued treatment

-Reduction of Blood pressure

Contraindications

-Allergy to any of the ingredients -Concomitant use with PDE-5 inhibitors such as tadalafil, sildenafil (Viagra) and vardenafil may lead to severe hypotension, syncope or myocardial ischemia.

-Concomitant use with «soluble guanylate cyclase stimulators» such as riociguat may lead to a violent drop in blood pressure.

Store at a temperature not exceeding 30°C in a dry place.
